Universal health care is an ostensibly bipartisan prerogative, but what it actually means and how it's achieved is a somewhat moveable feast. Spending, we are told, is unsustainable as the population ages and we move toward ever-more personalised and technologically-advanced treatment paradigms. The objective of this rhetoric is to rationalise the privatisation of our health system by stealth. The latest wages figures are something of an inconvenient truth in this 'unsustainable spending' fiction.
Last Wednesday, five Michigan officials were charged with involuntary manslaughter for their role in the unfolding health crisis in Flint, Michigan - a crisis that has included at least 12 deaths from Legionnaires' disease, in addition to the possible lead poisoning of a whole population. The people of Flint were aware that something was wrong from the moment their water was switched over to the Flint river in April 2014. They just couldn't get anyone to listen.
Next year marks four decades since promulgation of the seminal Declaration of Alma Ata, which declared health to be a fundamental human right and laid the foundations for what are now widely championed as the social determinants of health. Without action on the social determinants, health policy can be a little like that joke about the cyclopean orthopod who, when confronted with a patient suffering fatal internal bleeding, is interested only in fixing their broken leg. The Rio 2016 Olympics has earned a well-deserved label: the jogos da excludad, the games of exclusion. It is a label that shames a ruling class that got its priorities wrong. In the name of the Games, 77,000 residents of Rio's favelas have been evicted and hundreds of these settlements have been bulldozed. Those favelas that avoided the bulldozers have been hidden behind concrete walls that epitomise what theologian Leonardo Boff has called the 'lack of shame' living deep in the Brazilian soul.
My patients who earn $36,000 a year pay $36 for most prescriptions. My patients who earn $360,000 pay the same, and those on $3 billion pay the same. Usually, these prescriptions are for conditions which can't be avoided - it's just bad luck. This government imposed co-payment is a tax on illness. It is not noticed by those on $360,000 but for those struggling on $36,000, it does affect their small disposable incomes. It is a regressive tax, and its effect on patient behaviour is well documented.
Election times are full of sound and fury, much of it broadcast on a loop. But they are also marked by silences. These silences indicate concealed perils in society. Asylum seekers and young Indigenous people are spoken about, often noisily, but always as the object of policy, not as people whose lives have been blighted by policy. The human beings who suffer are shrouded in silence. This silence is an ethical silence that covers people whom we want to keep out of mind.
